Why can you tell them the truth without hold it inside?

lying about other your problem....
maybe.. they just dont want other to know and get involve of the problem they had....
or maybe... they are to shy to tell coz.... others might laugh at them and it adds up to the insecurity they have....
or maybe ... they are better off alone and solving it alone so that if worse comes to worse... they dont need to blame others or others blaming them.... in other words ..... PRIDE
but lying to yourself is kinda... different... you have so much pride or insecurity, that denies your own problem to yourself...
its pretty much on how they are raised.... and how they handled life situations before that pushes a person to make a barrier between others and him/her self

There is no point to lying to yourself because any decisions you make in life have to be based upon reality and that can not be done if you are lying to yourself. As far as lying to others, sometimes that may be necessary if it is something that is none of their business, something they can't help you with, or something that would only cause them pain to know.
