sacanagem can be considered a slang or a verbal insult. If it's used in a sentence which the person is trying to say something like "Damn it our car broke down!" In Portuguese "Damn it" would probably be replaced with "sacanagem" it can mean many things really, it doesn't have a specific meaning in English, it can be "damn it", "oh crap", "WTF?" and other actions like that. Now for the verbal insult the word changes a little bit and turns into "Sacana" instead. If someone comes to you and say "Voce e um maior sacana!" it means something like "You're such a douche bag!" somewhere between those lines.

Hope that helps! :D
