The word "Adapter" can refer to 3 different items:

Your charger is also an adapter. It takes power from your wall socket and transforms it into DC current that recharges your battery.

Another adapter is what connects to your cigarette lighter or electric power outlet in your car. This can recharge your phone while on longer trips. If you never expect to be in the car long enough to wear down your battery while chatting, then you may not need this.

And finally, some people refer to the USB connector to your computer as an adapter. Not all phones have this, but if you do, you can download pictures from your phone onto your computer, or install new ringtones from your computer into the phone. In addition, you can back up your phone's phone book so that you will always have a copy, just in case you lose the phone. Most people find this a real convenience and do want such a cable.

I hope this helps.

An adapter, as the name suggests adapts to the kind of socket you plug your charger in. There are different standards across the globe. US has flat pins, europe has round ones and so on... In short, no, adapter is not necessary with a charger.
