If both teams are going to the postseason (one as the NL wildcard), then a list of tiebreaker methods is used until the tie is broken. The first option is season head-to-head record, and the D'backs won it this year, 10-8.
If only one of them will be advancing to October, they have a one-game playoff. MLB held coinflips a few weeks ago, and for the NL West, the Diamondbacks would host the Padres if such a game is necessary.

It depends...
If both are assured of a playoff spot (division winner plus wildcard team), then the division title goes to Diamondbacks, as they have the better head-to-head record.
If more than one team are tied for the last playoff spot there would be a one game playoff round.

They'll both make the playoffs if that's the case, but they'll have to play a tie breaker for the division winner and the other team will get the wild card, but it's ok the rockies are going to win the wild card so it doesn't matter.
