The MD is a relatively low academic degree, it's called a "first professional" degree and is actually below a Master of Science for example.
Very many medical doctors get advanced degrees (MA, MS, DSc, PhD) in fields related to their research interests so that they can qualify to teach medicine or so that they can conduct research.
A PhD in Internal Medicine would not be anything unusual for an MD who teaches Internal Medicine to have. The question you want to ask is where is it from? The quality of the awarding school is the important consideration.

Yeah. The doctorate is awarded in medicine. That's the MD degree, and they're all the same. The internal medicine part is when you become a Fellow or Diplomate or something of the internal medicine professional society, the internal medicine equivalent of the American College of Surgeons.
Now, there are some schools which have, or had, some sort of a weird PhD in medicine. I know little about this, and I haven't heard of it in many years. But it still wouldn't have been in internal medicine, not in the US.
So what you've got there is either a foreigner, and God only knows what sorts of degrees they've got, or you've got someone who is trying to gild the lily a bit more than is desirable.
Go call the schools that he's listed on the resume. I don't like it.

As far as I know, a person can 'only' obtain a medical degree including MD or doctor of osteopathy, DO, as an internist (dr who specializes in internal medicine) by going to medical school. If you are looking for a shorter college program, perhaps majoring as a physician's assistant or (after obtaining a bachelor's degree as a registered nurse) nurse practitioner. There are some medical doctors that also attain a doctorate (Ph.d) in some type of research capacity or such. Some medical schools offer a joint major of MD/Ph.d.
