Registry cleaner are very dangerous. If you or the program make a mistake you could end up reinstalling windows. Most registry error are no more than dead regkeys that neither hurt nor effect your computer's performance. If you do not have noticable problems your best bet is to leave your registry alone.
If you insist on modifing your registry you need to first make a System Restore Point and the use a program like ERUNT to back up your registry. With this you at least have a chance of recovering from a damaged registry.

Good luck.

Since there is more to the question... I would suggest two online scans for trouble.

1) Trend: http://housecall.trendmicro.com/
2) Symantec: http://security.symantec.com/sscv6/default.asp?langid=ie&venid=sym

Run those test first and then worry about the registry after. I'm sure a lot of other people would agree about the workings of CCleaner: http://www.ccleaner.com/
I've used it for over a year and it's a great program with a lot of other neat features.

Also, if you should consider some anti-ware programs;
Spybot: http://www.safer-networking.org/en/index.html
Ad-Aware: http://www.lavasoftusa.com/products/ad_aware_free.php

I hope you also have anti-virus, if not;
AVG: http://free.grisoft.com/

Good luck on your quest!

Unless you know what you are doing, I would say: Don't bother. Those programs usually cause more harm than good. If your computer is working fine, then you don't need that program.
