Due to the HIPAA laws once you've signed a Release of Medical Records then the specialist must release the requested information to your GP. You can actually under the same laws go directly to your specialist and sign their release form and they are required to release the requested information (it will be a copy and you will more than likely be charged a fee for the copies.) If you go through your GP then he/she should be able to get them free of charge. If you or the GP continue to have trouble remind them that under HIPAA you are entitled to those records. You can go online and look up the HIPAA law so you will know exactly what your rights are and what you can do if they continue to refuse to send your records.

Pretty simple. Just go in and tell them what yo want, you will have to sign a from, but they should not be a problem since they are yours. If you werer in a hospital they will have a form to sign and probalby charge you a dollar a sheet. If you just want the part that concerns your physical condition then tell them this as if not we will include the dailey routine and nurses report and stuff like that.
It's not a big deal as when I get mind the form I have to use to request my records has a place on it to mark for self, relative, attorney, etc.

Legally you are entitled to your medical records. The office can charge you a duplication fee, but they can not with hold your records from you, even if you owe them money.
Keep calling until they get the records to you. You may want to try calling the office manager or the main administration office and find out how to file a complaint.
Unfortunatly a lot of offices drag their feet, and most often it is not worth it to get an attorney, but if you already have an attorney you may want to ask for their opinion.

You have to go IN and request them. You cannot request your records via phone. When you go in your will fill out a form and sign it stating you want your medical records. They will then make copies for you and either mail them to you or give them to you depending how they do it. They may charge you for making copies and yes that is legal. They cannot give your records just by you calling them as that is against the law and in violation of HIPPA. Go in to the clinic and sign the form.

Go to the medical records office, and they will charge you money but you can get them from them. You may have to go throught the business office first, though.

No , it is not. But it is typical. Tell them you will be
there on a particular day to pick them up personally. Also report this to your insurance company.
