For the most part it doesn't. The grades and course work from your undergrad program are more important. Make sure that your grades are good, your coursework is relevant, and you have extra-curriculars and internships that support your long term goals and you should do well.

Yes, very much. That is not to say that there aren't exceptions and that you can't get in if you went to a lesser school, but the probability of your getting in if you graduated from a rigorous program at an elite school is greatly improved.
