a menstrual period is when you bleed from between your legs, from your vagina. it generally happens once a month.
a first menstrual period is the first time this happens. doctors will ask this so that they know whether or not you have started menstruating. it basically mean 'have you started having your periods yet?'

It means your ovaries dropped a egg in to your fallopian tube and it did not get fertilized. On your period, the tissue it creates when the egg is dropped is breaking down
