After Iran retaliates the Bush administration will enact a police state and throw all dissidents into detention.
The entire middle east will erupt in violence. Israel, China and Russia will be involved.
Nuclear weapons will be used by the US first. The result will be WWIV.
Cheney will make sure this happens before he leaves office.

besides the nuclear winter and its associated problems, probably a lot less populated.
the usa will probably end up being more hated than it already is... the usa will over extend its reach, have hardly any manpower to fight on all the war fronts, americans will probably learn some humility when a huge chuck of its population will be vapourised.
with all of the above factored in, with out the usa the world will be a lot better off.

It won't happen while Bush is president.
Russia and China will NOT come to Iran's defense, they have nothing to gain by doing so.
Iran will strike the only country they can, Israel. Which, of course, would be the primary target of their nuclear weapons anyway..
