Maybe check into Mayhem, not so much what they did on stage but what they did off. The singer shot himself with a shotgun, and the first thing Euronymous, one of his band mates, did when finding the body was take a picture, that was later used as a album cover. Euronymous was later killed by varg vikernes, who was the bass player for the band at that time. Vikernes also set fire to churchs in Norway that where hundreds of years old.

Sex Pistols
Beastie Boys
Frankie Goes to Hollywood
Iggy Pop
New York Dolls
Sweet
Ozzy Osbourne
Jimi Hendrix
The Who

You could even include Elvis Presley who was banned from being shown on TV from the waist down when he first appeared because he was considered so shocking. The Who used to smash up their instruments and Jimi Hendrix set fire to his guitar and was associated with drugs.
Punk Rock was a major shock especially the Sex Pistols. Safety pins though the nose, bondage fashion, spitting from audiences, swearing on live television, controversial lyrics.

Dimmu Borgir's Enthrone Darkness effective. i think of that with track titles like Tormentor of Christian Souls and Spellbound (by using the devil) that they had truthfully run for the hills. Then as quickly as I see them working for the hills i could blast off Iron Maiden's "Run to the Hills". What a scene it will be!

I'd check out G.G. Allin. That dude was messed up.

It's a bummer that I can't think of their name, but a couple months ago I went to a friend's house and he was watching concert footage of a band where the singer kept vomiting regularly throughout the performance. I was even pretty grossed out inevitably and that's not easy.

Good call with Elvis, Floody. It's easy to take for granted now, but there is certainly something to be said for a guy who could only be put on television above the waist because they were scared he moved too sexually to be televised.

Marilyn Manson HAS to be in it. Plus, Donahue said he looks somewhat like Alice Cooper.
