There are a few possibilities.
1) Maybe he's tougher than he looks, and used his TK to keep himself from bleeding to death.
2) Maybe he absorbed the abilities of the cockroach that was in that scene; some insects can regenerate if you cut pieces off them.
3) Maybe he killed the woman that did illusions and took her power first. Then when he showed up, he made Peter look like him and himself look like Peter. When Hiro showed up, he stabbed Peter, thinking he was Sylar. Sylar used his TK to make everybody think it was him fighting back. (No visible special effects with the TK, remember.) When Nathan showed up, he flew off with Peter who was really Sylar. Then Sylar tok Nathan's power in the sky and blew up. Later, he would show up as Nathan, claiming that Peter pushed him away at the last minute so he could blow up safely but without killing Nathan. Now we have the situation that was shown in one episode, where Sylar was President and everybody thought he was Nathan. Sound reasonable....?

Who knows! I thought that at first until they showed everyone looking up at Peter and Nathan and not at Sylar. I figured they wouldn't give us the satisfaction of killing him off like that. It was too easy!

He was in close enough proximity to Claire to absorb enough healing power. It was shown that he dragged himself into the sewer.

he ecaped in a suer the scene was syler laying on the ground bloody then a open suer lid
