The meteor that killed the dinosaurs is not a natural disaster as it is defined as an event that effects humans. (Floods, Droughts, Pandemics, Earthquakes, etc)

The 1918 Spanish flu pandemic, killing an estimated 50 million people worldwide.

1931 China, Yellow river floods, Killing Millions.

Yeah, I'd say the dino's one as that wiped out at least 3/4 of all life on earth so pretty major. As for earth based only, well there was a global iceage but I can't remember exactly when - something like 200m years ago. Not to be mixed up with the much more recent one.

I believe there have been recorded floods in China which killed as many as a million people.

But at the other extreme, dry weather coupled with civic unrest in Africa have killed millions due to famine, an event which continues to this day.
