If it is decent hotel, all the room in the hotel in Tokyo are air-conditioned. A/C does mean not only cooler but also heater in Japan.
Rooms are usually centrally air-conditioned/heated. Some hotel have control panel in individual room. Then you can adjust the heating temerature.

Others are correct who said you need heater in Tokyo in December, not air-conditioning. Also, It's been years (long years, too) since I heard of any hotel charging extra for air-conditioned rooms.

Definitely not.
Tokyo in December is pretty cold so you will not need air conditioned. Instead, make sure the heat system works well in your room... but hotels in Tokyo are very good and the customer service in Japan is one of the best... whatever a customer needs the Japanese will provide... it is amazing!

When it's hot, it blows cold. When it's cold, it blows hot. You certainly don't want to open the window in central Tokyo at any time of the year.
