There are a number of causes two of which could either be his addiction to alcohol and/or medication if he is on any. Both these things doesn't necessarily cause no ejaculation but can cause a delay
First thing you probably need to do is to talk about it. Find out if there are any medications, alcohol involved or if there is another problem. Involve a medical professional if you think it is a deep seated problem
Meanwhile before sex make sure he is relaxed, not anxious or tired. Talk about what things get him off. What he does to himself that you could try. Get him to a point of no return with his or your hands and then have sex. It may be a pyschological hurdle that this may help him get over

Could be that after dating for so long and not having sex he has a mental block about it. He is used to masturbation and knows it is OK to have a full release that way. He may also be afraid of letting go because of pregnancy and other issues. More than likely it is a mental thing though. Something you might try I did not see on here is to have him masturbate until just before he is going to release then start having sex. Because of the timing he might have choice but to release and that may help him get through the mental block after a times of this.
The drinking may have something to do with it but you would have to wait a couple of days without him drinking in order for his body to flush everything out naturally. Then try seeing what is happening.

Have sex until you are finished and then masturbate him to a finish.
Does he finish if you do oral on him?
Maybe he gets off on pain since you said he is rough on it. Try biting lightly on it and see if that does the trick.

Sounds like alcohol is the primary problem. See if you can get him to stop drinking one night and see if you have the same problem. If not, then you know that you have to get him off the booze.

It's most likely a psychological thing...it is possible to see a counselor about this. Some women are this way, to, they can only get off manually, or orally.

he has a physiological issue, he seems to function OK until the both of you are naked, he should see a counselor or therapist. something in his past wont let him or allow him to relax & be comfortable with a naked woman..
