If I am moving house, changing broadband provider and changing phone number and name?

Do I just start again with a new provider?

In that instance no. You will not need a mac code. Dont forget,if your leaving in the middle of your contract you may have to pay the remainder of the time left. The mac code on the back of the modem, is not the one required by your new ISP anyway as stated by "curious in Ohio"

A MAC (Migration Authorisation Code) is for migrating your broadband connection from one ISP to another.

Since you will be connecting from a different phone line, you don't need a MAC from your existing ISP.

If you plan to keep the same ISP, it would be worth contacting them a few weeks before your move.

This is not the same as the MAC for the modem, this is a Media Access Code, this number is often on the base of the modem. Every network card has a unique MAC address.

I depends on what your planning on doing as far at networking goes. You will get a broadband modem from your Internet Service Provider. If you plan on hooking a router to split the one signal into multiple computers you may need the MAC address of the Modem, but it depends on the configuration of the router. Some of them detect the MAC automatically & others you have to enter it yourself. Either way the MAC address is normally on the Modem itself, so you would have the MAC code on the new modem.

The MAC Address is hard-coded into every network card. You will not have to do anything when connecting to another network. Its also called a 'Burned in Address' meaning it doesn't change. So you will keep the same MAC address.
