Try to talk to a doctor and get tested for ADD or something. Ritalin can help adults too, but here are a few other tips if you don't want to take it there.

1. Make lists. You feel more motivated every time you check something off.
2. Whenever you have an impulse to finally do something, just do it!
3. Seriously, if you find yourself masturbating more often than you like, stop it. It does in fact get in the way.
4. Get more than 8 hours of sleep every night.
5. Surround yourself with people who make you feel ashamed of your procrastination. Motivated people can help to motivate you.
6. Self-hypnosis CDs and tapes exist for people like you. As to whether or not they work, it is entirely up to you.
7. Take up a hobby, something that interests you. It'll keep your mind engaged and get you in a good mood to do other productive things.
8. Try to eliminate snacking and watching TV more often than you need to. These are major time-killers, and have negative effects on both your body and your mind.

No lie, you could have some kind of something. Ok, you could be a.d.d., have chronic fatigue, be eating the wrong foods, drinking the wrong drinks, not bathing enough, or you are just plain lazy. You are not lazy. Some people are procrastinators and cannot help it. I am the opposite, I have to keep things in order and get things done. My best friend is just the opposite, her home is so cluttered with art supplies that she will probably never use. Relax and be happy with yourself. You are good.
