I don't have an iguana, but I have a water dragon. They also have spikes on their backs. Our vet told us before that when they don't get enough nutrition they will start pulling the nutrients from their tails and spikes. Maybe that is what is happening. Are you feeding your iguana plenty of veggies and protein? Also it is good to use a calcium supplement. You should probably consider taking your iguana to the vet for a checkup.

the spikes on your iguana can grow back
as for why they fell off, there could be a number of reasons for that possibly getting stuck in a tight spot in your cage or maybe rubbing against chicken wire (if thats what your cage is made of)
