I have an ex that I think is trying to mess with my head...

If she gave you a copy, call that clinic or stop by and ask them if it is legitimate.
I worked at Planned Parenthood for awhile during my master's degree and we did actually have a Woman who had gotten a hold of our stationary and letterhead and was trying to scam men she had been for half the cost of an abortion by printing out a phony receipt. It was the first we had ever heard about it so you have every right to be suspicious.
I would advise you to call the clinic she is claiming performed the procedure or better yet stop by with the copy she gave to you to verify it. They won't be able to give you specific information about who has gone there but they will be able to tell you whether the copy you hold is legitimate or not.

It would probably look like a schedule card from an abortion clinic or doctor. But I agree, it sounds like you are planning on faking a pregnancy. Thats lame.

It varies from state to state and facility to facility. You would need to know what Dr the patient is seeing and the varying forms used by a given clinic. Bottom line there is no real answer to your question.
