Since this is your 3rd or 4th question on html might I suggest that you actually learn how to do whatever you are trying to do by buying a book or going on line and studying the subject.

That is not a put down but how I learned.

Many people, new to web design, believe that YA is the place to have the codes written for them, preferably by pros.
IT IS NOT THE CASE: we are not here to write code for you, nor are we are here to do your homework!
(I do report these abusers...)
You MUST learn a bit by yourself, PROVE that you have tried. AFTER, we will help you!
So, stop asking beginner's questions, and start learning.
Go to www.w3schools.com and start reading, start experimenting, start CODING.
When you hit a bug or something you do not understand, call us: THIS IS WHY WE ARE HERE.

Well that ones for username and password etc.

BINGO JP! you'll likely also favor to go back pretend decrease back to the onsubmit handler or only upload it after the decision to redirect() to save the shape from actual filing. both: onsubmit="go back redirect();" and then go back pretend from the function or: onsubmit="redirect(); go back pretend;"
