It is increasingly harder to tell the differences, but the main differences are that Neo-Conservatives support the welfare state (whereas real Conservatives are against most welfare), Neo-Conservatives support open borders (whereas real Conservatives do not), and Neo-Conservatives want to create a dictatorship at home while waging wars on numerous countries (Real Conservatives don't support wars, except for defense; Real Conservatives though, may be fooled into believing that Neo-Con wars are "defensive").
Examples of Real Conservatives would include Ron Paul or Pat Buchanan. Examples of Neo-Conservatives would include Dick Cheney and Rudy Giuliani.

Neo conservatives are the liberals of the Republican party - Bush, Rumsfeld, Gonzalez, etc. Real conservatives believe in small govt, personal responsibility, no welfare state. Examples are Ron Paul, Tom Tancredo, Teddy, and Ike.

There is no difference what so ever. The word neo means new. neo-cons are new conservatives. They used to be mostly democrats.
