I am an existing AT&T customer.
I have a family plan with 4 lines.

I bought one iPhone for one line a couple months ago.
I kept the existing family plan that I had.
All I did was add the iPhone data plan for an additional
$20 a month.

Now I want to buy another iPhone for another line.
Which would make a total of two iPhones on
one family plan.

Would I have to pay an extra $20 a month for the second
iPhone?
Or does the existing iPhone data plan apply to all iPhones
on the family plan.

Since you have two iphones on your family plan, A T&T will charge you a extra $20 for the data plan for each phone. That is because each iphone has a different telephone number and a data plan for each phone. It wouldn't make sense for A T&T just to charge you for $20.00 data plan for all the phones. Remember that cell phone data rates doesn't come cheap.
