Like for instance if I were going to be a exercise counselor would I need to do anything to have the title "counselor"

In many countries and states, specific titles are reserved as a matter of law to licensed professions.

Some states reserve "counselor" for someone who is either licensed to practice law, or licensed to practice mental health care (social worker, psychologist, family therapist, etc.)

Some states are very broad in their limitations, some are very specific and allow variations like you describe.

Check your local listings.

you can call yourself a banana counselor and it is fine. But if my assumptions and hear say are correct you can't charge for services without a license.
[EDIT] This is the law in Nebraska.
