I want to completely format a DVD disc and start from scratch.

If the DVD is RW just right click the unit and then Format.

Provided your DVD is DVDRW (Read and write) you should not have a problem reformatting.

You can simply right click the unit and then select Format. or use one of your DVD copying favorite copying programs. Most have an option that will allow you to reformat a CD or DVD prior to using it again. Since you haven't indicated which brand you use, I won't be able to include the instructions. But check under the help section of your CD/DVD burning application it should give you instructions on reformatting as well.

Don't bother.
Destroy it completely and toss it out. Literally break it into small pieces and junk it.

DV-RWs (or CD-RWs), despite being rewiteable are not a good investment - they degrade too fast, and are prone to errors.
