You can call your local SSA office and ask what the requirements are such as birth certificate only and/ or State ID. If you were born here, then I cannot forsee you needing another form of ID to get a replacement. However, it would have to be an original copy it cannot be notarized or a photocopy

For a replacement card, proof of your U.S. citizenship and age are not required if they are already in our records. Under the new law, only certain documents can be accepted as proof of U.S. citizenship. These include your U.S. birth certificate, a U.S. passport, a Certificate of Naturalization or a Certificate of Citizenship. If you are not a U.S. citizen, different rules apply for proving your immigration status, and those rules have not changed.

If there's a Federal development on your city/city the place the Social safety place of work is generally placed is the place you may get carry of a replica of your certificates. The Social safety place of work may well be placed in a different branch so this is beneficial to envision the telephone e book for the area and make contact with quantity.

Go to www.ssa.gov. They will tell you how to get a replacement. On the upper right of the page type in lost card. The first answer is how to get a replacement SS card.
