The health department regularly inspects all restaurants. They have to pass the inspections or be shut down.
There is no government agency that monitors people and tells them what to eat, though.
By the way, a burger and fries once every two or three months isn't going to give anyone a heart attack. It's eating them too often that's bad for your health. People should eat responsibly...it's NOT the fault of restaurants to teach people good nutrition and refuse to serve people they think are eating too many unhealthy foods.

well, I believe you are starting of with a couple of incorrect assumptions, with a healthy dose of bias in your question.
First off - to say that fast food franchises are obsessed with cleanliness is not necessarily accurate. As in the case of most things - the average is, well, average. I would say they are no more obsessed than any other food establishment.
Second - how nice to toss out the contentious "product is toxic" phrase. Sure, fast food is far from the healthiest option, but toxic? Rein in the propaganda there a little bit.
As for the question as to why people don't freak out that they are getting cardiovascular disease, as many do when someone gets e coli . . . well, if folks could see the effects of cardiovascular disease more quickly, perhaps they would 'freak out.'
Why do people eat fast food so much? I don't know. Why do people smoke? Why do people drink? Why do people have unprotected sex? I suppose most folks just don't think about the 'big picture' and do what they want, rather than do what they ought.

I read your question and then reread your question and I began to get a little angry at it. My husband is a manager at a fast food restaurant. I used to work in the office of the franchisee of that restaurant. While it's true that the establishment must be exceptionally clean based on state and federal standards (Not to even mention corporate standards), the food that they serve is not toxic if you eat it in moderation.
I get all the stories of the people that go in everyday for lunch and order the double burger with extra cheese, an extra large side of fries or onion rings, and an extra large Coke.. Or funnier yet, an extra large Diet Coke...and these people are morbidly obese. I highly doubt that these people just eat this one big meal all day. There's also breakfast and dinner that I'm sure they're overeating with. A person will die if they eat that meal, a Grand Slam for breakfast, 2 Big Macs for dinner and a package of Oreos with a quart a milk before bed.
And if you'd like me to get started on it as well, every restaurant has healthy options on the menu. There are salads with low fat dressing options, there are grilled chicken sandwiches, there are whole wheat buns...so don't give out the impression that everything a fast food restaurant serves is bad for you. It isn't. So in essence, by the fact that they have those items on the menu, they are trying to protect people from "life threatening heart attacks."
Bottom line: Yes, burgers sell better than salads do. Fried chicken sells better than salads do. But it's up to the consumer to make the healthy choices.

People that care do avoid the restaurants you are talking about. They educate their children and do not spend their money there. I have a 4 yr old and we were at the park last week. One mom asked her little one if he wanted to go to McD's for lunch. Another child, about 3 yrs old said "gross, McD's is bad food and it's bad for you!". Clearly, his family does not go the fast food route.
Many people don't care, don't believe, or don't see other options. Not many places can provide a hot, reasonably (and debatable) palatable meal for less than $5. If youdon't have the resources or the knowledge of basic cooking, you wouldn't know how to prepare are meal for such a low cost. A brick of ramen noodles is no more nutritious and you get tired of that really fast.
Sadly, some people don't think long-term or much past the next day.

Well the main reason because the can be shut down for not having a clean restaurant and properly handled food methods.
As far as they are not concerned, well it is all about making money and even though most of the food is not healthy to you it doesn't have dire health effects unless you consume large quantities. Like anything in life you should do everything in moderation. I recommend you watch a documentary that you can rent at your local store called "supper size me" they have a guy on their who eats no less than 2 big macs a day and has being doing this for years and he is skinny and in perfect health. It is all about how active you are too.

I don't know! I wish more people would go out on the streets and have these fast food joints close down because all they generally serve is processed crap. The only exceptions to this are In&Out which serves the best quality burgers and french fries cooked in 100% all-natural vegetable oil and McDonald's which serves the grilled chicken wraps. If it weren't for the grilled chicken wraps, I would like to see McDonald's close its doors too because their burgers, fries, and apple pies are not good for anyone to eat.
