What is the penalty for mistakes that caused years of misery (harassment, invasion of privacy, ostracism, discrimination, emotional breakdown of the victim) or even death?

secret agents die if they make the big mistake. as for what you are inplying they simply pass information on to their section leaders who follow up on it. Agents rarely do public trails else they would be no longer "secret".
