Using pretty in front of an adjective is a figure of speech. It's being used as an intensifier (don't know if that's the right term) like "very" or "really". Saying that someone is pretty ugly is saying that the person is beyond being just a little bit ugly.

Simple. Ugly is the adjective of that someone. Pretty is the addjective of ugly. Hence pretty ugly means several times ugly or very very ugly. Just like your avatar.
