He doesn't want to pay support anymore and I don't want him to have custody or visitation, so we are both in agreement to terminate his rights. Where can I get the forms for him to do this? He lives in CA, where the custody and support orders are, but my daughter and I have lived in AZ the past 7 mos.

You need to see a lawyer or go to court

It will be much more difficult than you would think for the childs father to legally relinquish his rights even if you are now both in total agreement.

You may consider establishing residency in AZ and formally request court jurisdiction be transferred from CA to AZ, and if father does not object it will likely happen. Either way talk to a family law attorney about the legal pros and cons of what you want to do and how to do it.

There are long term effects of a decision such as this that may be more harmful to the child in the long run. That you may wish to discuss with a therapist who is a licensed psychologist. Then there is nothing to prevent dad from contacting his child after child turns 18.

side note if you seek the assistance from the state for support of the child, a release from your ex will not remove his responsibility to pay for the child as such the state will still pursue him for monies paid out

You HAVE to do this through the court. If there is no dispute, the court clerk can probably help you with the paperwork.
