The Movie Fu-ck
It uses the word "fu-ck" more times than any other movie ever made, 824 times in 93 minutes, for an average of more than 8.8 "fuc-ks" per minute. The DVD release includes an optional "curse counter" function that keeps a visual record of the number of appearances of the word both verbally and visually in the film.

As far as a nationally distributed film that was very popular and was just recently ruled as the most offensive film of all time, it would be The Big Lebowski.
One of the religious watchdog groups just did an analysis of the filthiest films of all time in tv and movies and The Big Lebowski won hands down for the most F-bombs (283) and content.
That's according to the group, but what a great movie. How did they ever edit enough of that film to put it on TV? They must have shot 2 versions or something!
South Park: The Movie came in 2nd.

I have no clue what the answer to this one is but I will say Scarface is up there.
Boondock Saints has quite a bit but I doubt it is up there compared to others.
