The mandatory helmet law was enacted on October 5, 1979. All players not signed to their first NHL contract by that date had to wear helmets. Any players who had signed an NHL contract prior to that date had the option to play without them. MacTavish signed his contract in April 1979

Randy Carlyle and Brad Marsh were the 2nd and 3rd last players to play without a helmet. In the 1992-93 season only Carlyle, Marsh and MacTavish had helmet waivers. Carlyle and Marsh retired at the end of that season. Robinson retired at the end of the previous season and Lafleur even before. that.

Craig MacTavish for the New York Rangers up until the New York-Vancouver series was over (NY 4-3, June '94). The NHL enacted the mandatory helmet law during that summer.
