Wear flesh-colored clothing, paint some of that "Easter basket grass" black and attach it to your ankles, and draw a small line on your forehead...

I personally don't involve myself with Halloween, however I love costumes. I just went to Dragon*Con, got lots of ideas from it.
Some costume ideas for Bald males, Capt. Picard, Professor X, and as you said Mr.Clean are 3 that come to mind.

I LOVE Halloween (so does my husband...he's 51 and I'm 48 and we spend all year anticipating it like two kids!!!!)...
Some friends one year, came to our annual party dressed in jeans, white t-shirts with a large letter "P" on them, and had their eyes blackened...they were "black-eyed P's"....LOL!
You could go as:
A bumble bee
A "Conehead"...(from SNL)
Yule Brenner from the King-and-I (actually, Yule Brenner in ANY movie!)...
Kojack (all you need is a suit, a hat and a Tootsie Pop and walk around all night saying "who loves ya baby"!)...
Depends on the age (and sophistication...or lack thereof!!!) of the crowd whether anyone would understand some of these!

Any costume he wants is good. Why would he be limited just because he has no hair? If the lack is an issue there are always wigs. PS; ITHINK BALD MEN ARE SEXY!!!
