As in how can a person know that everything they experience is not just a figment of thier imagination. The mind is a powerful thing and I don't have any doubt that it's possible that a person could fall into a sort of dream that they couldn't tell wasn't real. If they were in that dream for so long how would they even remember what the real world was like. It's sort of like asking how would a person in the matrix know they were in the matrix unless someone told them and took them out of it. But with psychosis it's just you, which would make it all the more sad.
