what does it mean?

Kinda depends WHAT is saying it, sounds like a wireless network PC has moved too far from the source of the connection, so can see it but not reliably connect, hence "Signal" is "out of range"

It means that the resolution and/or refresh rate are too high and the monitor can not cope with the signals being sent to it.

You need to reduces the desktop resolution or reduce the vertical refresh rate. (set it to 60 Hz for LCDs 75 Hz for CRTs)

If you can not see the screen and a reboot does not fix it then you will need to go into safe mode by pressing F8 during Windows startup.
