My goodness, whatever happened to people taking charge of their own lives?

If I've forgotten I always look for mail from the previous address, or letters I kept, credit card statements, or places where my account has not been updated and has a previous address. As for the dates, you'll probably have to guess.

Your credit history will have it.
You are entitled to one free credit report each year from all 3 major credit agencies.
Go to http://www.annualcreditreport.com
Fill out your information and you will get a complete credit report that includes all previously reported addresses.

If in the UK then check with the voters roll or your council tax details, or DSS details.
