Taking a year off can be extremely beneficial. Chances are, you have been working for four years for your undergraduate degree in some hard science, and for thirteen years of grade school before that. You deserve a year off!
Medical schools also love to see well-rounded applicants.

No, you talk to the schools about applying for the year you want to enter while you are still in college. Depending on the school, you are admitted to the school and then given deferred entry or you are evaluated for the next class since you will be out of the country.
