Well, think about it for a minute....If you just picture it in general, there are so many things that are freakin hilarious about calling someone a douche...It's a funny sounding word, it's a funny thing in general when you refer to a person as one, a douche even LOOKS funny!! I'd venture to say its origins probably started with a pre-teen boy, who maybe saw one in his house belonging to his mother? It probably was first used in some way to connotate the person was a mama's boy and/or a weakling (ie-a pansy a**, a sissy, a panty waist....all those good old words that have been used throughout history). Just a wild guess....IDK...but guys have a way of using funny methods of referring to each other as being "female" in some way, from sissy, to douche, to "you throw like a girl", to insinuating the other guy wears women's undergarments (yes, sad to say, I have seen this done many times! lol), to flat out saying the other person is gay.
Most of the time it's harmless, intended only in the spirit of fun and joking, so I don't really worry about it! LOL
And before anyone goes off on me, my uncle really IS gay, and probably tells better gay jokes, and says more funny remarks about people being feminine than anyone else I know!

That D sound makes for a good insult like a strong B sound at the beginning of a word.
The funny thing is i remember a time where scumbag made it past the TV censors, but they wouldn't allow douche bag.

Why did it get started? I think it's really similar to calling a guy a dildo. A man who is used by a woman and disposed of, who is useless outside of a specific service to be performed.
It's not one I've used much, though in my schooling we called each other dildoes quite frequently.
Adds: I think douche connotates sleaziness and dildo connotates idiocy. Other than that I think they have a similar meaning. It's interesting to see how often it happens that words like this enter slang.

Ha. Funny. I don't know, but the word annoys the s.hit out of me. It's just another way for a man to "feminize" and thereby insult (because we all know what an INSULT it is to be a woman or even to be "womanly"!) another man by claiming he's a feminine hygiene product. It plays on fears surrounding stereotypical masculinity, which is enforced by heckling from other males and homophobia. It probably originated in a men's locker room. Why don't we call each other jock straps? Male hygiene products are not insulting. "OMG, you're such a nut-sack." Yeah. Probably not. Yet just about every piece of female anatomy, every female hygiene product, every piece of female-specific clothing, and everything pink is insulting and laughable.
I don't know why people can't come up with more creative and fewer childish, juvenile, socially offensive insults.

People have a need to insult others and used terms that are not profanity but in their minds are undesirable names, I do not mind, I have been called worst.

Probably for the same reasons as "scumbag"- it's icky and has to do with our naughty bits. It's kinda fun to say, too.
