Between 4-6 weeks, if you have an ultrasound. They can see the hearts beating.

Your first ultrasound they'll check for 2 gestational sacs (around 5 weeks). The next step would be listening for 2 heartbeats around 12 weeks. Sometimes the heartbeats can be in sync and they may be hiding one behind the other. But as they grow, you will be able to tell for sure.
Some people say the doctors know by your HCG levels - this is NOT true. While higher levels can indicate multiples, they prove nothing. My first HCG level was 815 at 4wks. At 5wks it went up to 13249. Everyone was positive I was having twins when lo and behold, only 1 baby in the ultrasound. The doctor said that these numbers have very wide ranges and anything can effect them.

I had a early vaginal u/s at 5-1/2 weeks as I was on fertility pills to see if there was more than one baby.

Hormone levels do not always indicate multiples. My levels were checked at almost my 6th week of pregnancy, and they did not indicate twins. I had an ultrasound performed at 8 weeks and 3 days, and I saw both babies and heard both heartbeats.

My cousin found out she was having twins when she was 16 weeks. They couldn't find the sex till she was 24 weeks.

as early as four weeks. my doctor checked me at my first appointment at 8 weeks but i had a report from another doctor er visit due to cramps at 5 weeks. they knew at 5 weeks i was having one

They can tell by your HCG levels (hormone levels) within the first few months
