It's the same thing as a "Nancy Mary" or saying that some guy is just a big ole girl. It's a nice way to say a guy is suspected of being gay.
No big deal.
If you're a girl the reference is sometimes, "her name's Billy Joe Badass." Instead of dyke or butch.
