I did it and it cost me nothing but i did it at the time i got my divorce. You have a certain amount of time to do this in and i don't know if every state does it. I lived in Maryland when i had it done.I went to city hall marriage records.

There's no cost other than getting a new drivers license. You'd have to get new checks at the bank. I think your new SS card is free, although there may be a small fee. Then there's the passport that has to be changed.
Don't forget about your work related stuff like retirement account info and even e-mail address.
You can see why I just left mine alone.

First you have to go to the social security office and change it there, then you have to change it with DMV (oh by the way you don't HAVE to go through the court.) You have to change it on all of your bank accounts and financial holdings, any credit cards etc. You have to change it on all of your tax papers, as well as with your employer. You have to change all insurance papers as well as medical and dental records, car insurance, car loans and registration if the car is in your name. If you have children you will have to change your name on all of their records (school, medical/dental etc) You do not have to pay to change your name, you do not have to go through the court, you don't need a paralegal or go to a notary. Those who say you do, have never been in the situation.

Try a Paralegal Service in your area it is cheaper than a Lawyers fee. Otherwise just change it on your own, you just need proof of your Divorce and your birth certificate to change it back.

Contact an attorney who deals in family law matters and they can get the paperwork started.

You should have asked for it in your divorce proceedings. That's the way I've always known people to do it. I did it like that.
