Its a synonym of starving (without food for a long enough time to be fatigued and possibly near death, usualy used to exagerate hunger)
Also can be used as a synonym of "starved to death" like a dead body of a dog where the ribs showing I think might be considered famished.

As everyone else said, it means very, very hungry. Just remember that is is related to famine, which means a sustained period without food. A good synonym for famished is ravenous.
