I have seen several sentences in english where some writers use to write "had" twice as well as simultaneous. I am a bit confused that when shall we use had twice in grammer.

"Had had" is the past perfect aka pluperfect. The past perfect is used in reference to an action completed before another past action. In the case of "had" it's normally a possession rather than an action (though "to have" can also imply an action).

I had had my car for four years before I ever learned to drive it.

I had had a bath, but I didn't feel clean, so I had a shower.

Try to punctuate this sentence so that it makes sense: (while simon had had had sally had had had had had had had had a better effect on the teacher)

"Have you had lunch?"
"Yes, I have had lunch."

The combination "have had" is fairly common in speech. However, what if you were telling a story about something that happened a few days ago.

"Had you had lunch?"
"Yes, I had had lunch."

In English we use verbs carefully to explain when something happens, as if time was all that mattered. We sometimes use three or four verbs to get the point across as in "I would have been going next Tuesday... but I got sick."

Answer to the puzzle:
While Simon had had "had", Sally had had "had had". "Had had" had had a better effect on the teacher.
