"alien" describes anything foriegn to a place, be it from outside the planet or from outside the country. Not all aliens are portrayed as ugly in science fiction. This is much more your opinion than anything else. I myself find slimy green men ravishing.

the word alien goes back centuries. You may find it in the english translations of the bible, in legal documents predating the establishment of several countries, and in romantic novels and historical texts from the library of congress, all predating the Sci Fi culture we know today.
Alien really just means "not from around here" like the prefix Xeno- and its similar friend Exo. Ants have an exoskeleton, while we have endoskeletons, meaning the ant's skeleton is outside.
if aliens looked like humans, it would not be a terrible surprise: the important thing is that they are from elsewhere. Alien figures tend to be ugly because sci fi art was inspired in part by theories of evolution, and in part by horror novelists such as H.P. Lovecraft. Lovecraft wrote books filled with godlike aliens from other dimensions. Sci Fi writers, in general, try to guess what a person would have to be built like to survive in strange places like a red sun, a high gravity planet, a place with lots of radiation, or in a place where the atmosphere might be very dense.
More recent inspirations came from the Deep Sea - such as jellyfish, and from variations of art styles such as Abstract, and Dadaism.

What does the appearance of an alien have to do with the word that is used to label it? The word alien simply indicates that the being is from another place. What does the appearance of an alien have to do with anything?
