In other words, can a site slip their address into my history or does someone actually have to visit it for it to show up?

100% someone had to visit that site. You can get a better look at the other sites visited by searching the history files
In Internet Explorer

1. Open Internet Explorer.
2. Choose Internet Options from the Tools menu.
3. Click the General, Under Temporary Internet Files, Click Settings.
4.Click View Files

Search from the list of files adult content, cookies, search files and viewed websites.

You can record all your internet history. Using a monitoring software. Check out the link bleow.

Yes, cookies and some temporary internet files are a different story, but if it is in the history, then someone was there.

History yes.. it has been visited.
If its just a cookie, you can get it without visiting the site.
