The lowest altitude at which rain droplets form would be about 3000 feet above sea level. The highest would be about 35 000 feet. Above this altitude the temperature never gets higher than minus 40, and below this temperature you can't get liquid water droplets forming; even very clean supercooled water will freeze. But cumulonimbus clouds can go up to more than 60 000 feet, and hailstones can form at this height. This is higher than passenger aircraft fly; only the Concorde and military spy planes go higher. But raindrops reach their terminal velocity after falling only a few feet, so it doesn't make much difference how far they've fallen. They all hit the ground at pretty much the same speed.

It falls from where ever the raindrop (or snow/ice) formation is caused within the cloud to the ground. This is normally not from the bottom of a cloud but more likely in the middle to upper portions of most clouds.

In the case of drizzle, the very small raindrops can form just a few dozen feet off the ground.

In the case of desert thunderstorms. The base of the cloud may be in excess of ten thousand feet with the formation of the rain/ice/snow drops in the 25 to 30 thousand feet range. In the desert southwest, it is not too unusual to see the formation of rain/ice/snow even at or above 30 thousand feet and due to the deep dry air near the ground, the raindrops evaporate before reaching the ground. (Referred to as a dry thunderstorm.)

depends how high the clouds are at the time. rain clouds are usually quite low and heavy for the rain to fall
