yes it is necessary to submit a change of use application for the change from commercial to residential - and also a planning application for any changes made to the property - these can be done at the same time.
It does not effect it that a similar thing has been done in the past.

Yes you would have to have the property officially changed back. There is usually no problem with this, especially if it has been residential before. It used to be quite low cost formality where your solicitor applied to the local authority for change of use. There is probably a form available from your local council.

If the structure went through the proper municipal channels to convert it into a business, then yes. If it was done on the sly, no as the municipality still thinks it is a residence.

ITs always best to check with your local governement on this. It does have to be zoned residential, but each state/local government does this differently.
