Best case scenario??? You are a gentleman, politely put the napkin back on the table and ask for a check. Do not make yourself the ridicule. Whether she is right or not shouldn't matter, it is whether or not you act like a gentleman afterwards.

Rude gestures are never OK. Obviously you thought something was wrong with it because it made you raise an eyebrow. I would have ignore it, order the check and left.

You two need to discuss her behavior, and the inappropriateness for it...

If she does it again, that shows lack of self control or respect or both in which, maybe you need a new girlfriend...

It depends on what kind of expression was on her face when she threw it. If it was a smile, of course she was kidding. If it was a look of mere disgust, well then I would say that really sucks.
