I would also agree with ich.

Is that a green spotted puffer? Do you have him in saltwater? (despite how they're sold in stores, these fish are high-end brackish and need almost marine conditions as adults. The salt will also help with the ich.

Looks like a bad case of ich if im not mistaken...you;ve just reminded me of rusty...tear...a puffer that will be missed
