My wifes breath smells like she used diarreah for mouthwash and her b.o. makes my eyes water. I keep avoiding sex because I cant get wood under these circumstances. Our marriage is suffering, should I file?

No you can't file or you will be breaking your marriage vows, you remember them? For better or WORSE, in SICKNESS and in health, for richer or POORER, until DEATH do us part.
Get a pretty basket and fill it with dental flaus, toothpaste, mouth wash, bubble bath, sweet smelling soap, a body scrubber, anti-persperant and put a big pretty bow on it and give it to her, when she asks you why, just tell her you love her.

you buy her products for personal hygiene then tell her with all honesty about what you feel. there should be no other people who should know it first other than her. encourage her and appreciate her if she has improvements. don't make her a center of joke among family and friends. this is a concern for couples only. be gentle to her. probably, she became like that for she has been busy making your family life a comfortable one. it's time to give back something to her. spend more time with her... remember, she is your other half.

I don't know the details of your marriage, but a lot of that depends on why you married her in the first place.
Hygiene can be corrected without having to go to much more serious measures.

Put some cologne on yourself, thick enough so that it blocks out her BO, then do her doggie style so that she dosn't breath on you.
Or drink about 10 beers tonight with cool ranch chips, don't rinse or brush before bed, when you wake up tomorrow blow your breath on her and say see this is what you smell like

Take a deep breath and go talk to her and tell her that she doesnt smell pretty, and how it doesnt turn you on. Be honest with her.
