It probably is a normal variation but if he has tendersness or if the size difference is 2-3 times and getting larger he may have a hernia that is causing fluid buildup around the testes. I would have it checked by his family doctor if the size difference is that significant. At the very least you get piece of mind...

This is not uncommon, however, men should do a testicle self exam monthly just as women do a breast self exam. It is usually most comfortable in the shower, where the warmth will have everything loosened up. He (or you!) should gently palpate (feel) every area of each testicle for lumps, swelling or tenderness. Any abnormal finding should be examined by a doctor.

unless your talking a huge difference, like one's the size of a hand fruit and the other like a grape!! for most guys one testicle is larger and hangs lower than than the other. 99.9% perfectly normal. if he or you feel any bumps or tender areas then he should get checked out, but just by the size and dangle, nothing to worry about.

No swelling, pain, or tenderness? Normal! Testicles are supposed to be assymetrical (uneven) to allow them to move around without "bumping" into each other.
